ฮอร์โมนอินซูลินมีความสำคัญต่อร่างกายอย่างไร
อินซูลินเป็นฮอร์โมนสำคัญตัวหนึ่งของร่างกาย สร้างและหลั่งจากเบต้าเซลล์ของตับอ่อน ทำหน้าที่เป็นตัวพาน้ำตาลกลูโคสเข้าสู่เนื้อเยื่อต่างๆของร่างกาย เพื่อเผาผลาญเป็นพลังงานในการดำเนินชีวิต ถ้าขาดอินซูลินหรือการออกฤทธิ์ไม่ดี ร่างกายจะใช้น้ำตาลไม่ได้ จึงทำให้น้ำตาลในเลือดสูงมีอาการต่างๆของโรคเบาหวาน นอกจากมีความผิดปกติของการเผาผลาญอาหารคาร์โบไฮเดรตแล้ว ยังมีความผิดปกติอื่น เช่น มีการสลายของสารไขมันและโปรตีนร่วมด้วย
อาการของโรคเบาหวาน
คนปกติก่อนรับประทานอาหารเช้าจะมีระดับน้ำตาลในเลือด 70-110 มก.% หลังรับประทานอาหารแล้ว 2 ชม.ระดับน้ำตาลไม่เกิน 140 มก.% ผู้ที่ระดับน้ำตาลสูงไม่มากอาจจะไม่มีอาการอะไร การวินิจฉัยโรคเบาหวานจะทำได้โดยการเจาะเลือด อาการที่พบได้บ่อย
คนปกติมักจะไม่ต้องลุกขึ้นมาปัสสาวะในเวลากลางดึกหรือปัสสาวะอย่างมากไม่เกิน 1 ครั้ง เมื่อน้ำตาลในกระแสเลือดมากกว่า180มก.% โดยเฉพาะในเวลากลางคืนน้ำตาลจะถูกขับออกทางปัสสาวะทำให้น้ำถูกขับออกมากขึ้น จึงมีอาการปัสสาวะบ่อยและเกิดการสูญเสียน้ำ และอาจจะพบว่าปัสสาวะมีมดตอม ผู้ป่วยจะหิวน้ำบ่อยเนื่องจากต้องทดแทนน้ำที่ถูกขับออกทางปัสสาวะ อ่อนเพลีย น้ำหนักลดเกิดเนื่องจากร่างกายไม่สามารถใช้น้ำตาล จึงย่อยสลายส่วนที่เป็นโปรตีนและไขมันออกมา ผู้ป่วยจะกินเก่งหิวเก่งแต่น้ำหนักจะลดลงเนื่องจากร่างกายน้ำน้ำตาลไปใช้เป็นพลังงานไม่ได้ จึงมีการสลายพลังงานจากไขมันและโปรตีนจากกล้ามเนื้อ อาการอื่นๆที่อาจเกิดได้แก่ การติดเชื้อ แผลหายช้า คัน คันตามผิวหนัง มีการติดเชื้อรา โดยเฉพาะอย่างยิ่งบริเวณช่องคลอดของผู้หญิง สาเหตุของอาการคันเนื่องจากผิวแห้งไป หรือมีการอักเสบของผิวหนัง เห็นภาพไม่ชัด ตาพร่ามัวต้องเปลี่ยนแว่นบ่อย ทั้งนี้อาจจะเป็นเพราะมีการเปลี่ยนแปลงสายตา เช่นสายตาสั้น ต่อกระจก น้ำตาลในเลือดสูง ชาไม่มีความรู้สึก เจ็บตามแขนขาหย่อนสมรรถภาพทางเพศ เนื่องจากน้ำตาลสูงนานๆทำให้เส้นประสาทเสื่อม เกิดแผลที่เท้าได้ง่าย เพราะไม่รู้สึก อาเจียน น้ำตาลในกระแสเลือดสูงเมื่อเป็นโรคนี้ระยะหนึ่งจะเกิดโรคแทรกซ้อนที่เกิดกับหลอดเลือดเล็กเรียก microvacular หากมีโรคแทรกซ้อนนี้จะทำให้เกิดโรคไต เบาหวานเข้าตา หากเกิดหลอดเลือดเลือดแดงใหญ่แข็งเรียก macrovascular โดยจะทำให้เกิดโรคหลอดเลือดหัวใจตีบ อัมพาต หลอดเลือดแดงที่ขาตีบนอกจากนั้นยังอาจจะเกิดปลายประสาทอักเสบ neuropathic ทำให้เกิดอาการชาขา กล้ามเนื้ออ่อนแรง ประสาทอัตโนมัติเสื่อม
ใครมีโอกาสเป็นโรคเบาหวาน
สาเหตุของการเกิดโรคเบาหวานยังไม่ทราบแน่นอน แต่องค์ประกอบสำคัญที่อาจเป็นต้นเหตุของการเกิดได้แก่ กรรมพันธุ์ อ้วน ขาดการออกกำลังกาย หากบุคคลใดมีปัจจัยเสี่ยงมากย่อมมี่โอกาสที่จะเป็นเบาหวานมากขึ้น ปัจจัยเสี่ยงที่จะเป็นเบาหวานได้แสดงข้างล่างนี้
ใครที่ควรจะต้องเจาะเลือดหาโรคเบาหวาน
